The host employers (with Dun & Bradstreet identification number) will need to conduct on-site supervision, monitoring and evaluation to assure the participants to implement their TRAINING/INTERNSHIP PLACEMENT PLAN, i.e. DS-7002. The periodic written evaluation at minimum at the mid-point and the end of the program will need to be submitted from host employers regarding each participant’s performance, including the number of hours performed, the type of training, and the quality of the performance.
